2026-09-20 UTC · Filing-link destination repair
Release: cpm-release-hygiene-v1-20260920. Affected material: hospital-record filing citations in the rich CMS record dataset.
What changed and why: 3,975 citation instances, representing 671 unique EIN-and-object-ID filing destinations across 795 rich hospital records, now open a full-filing view at ProPublica. The retired IRS per-filing e-postcard URL is retained in the dataset as audit metadata. The destination changed because the retired IRS pattern no longer resolves reliably.
Data effect: none. No filed value, displayed amount, calculation, hospital-to-filer relationship, filing year, ranking, or eligibility threshold changed. Labels now identify ProPublica as the filing-view host while continuing to identify the IRS form section being cited.
Evidence and verification state: every destination was constructed from its exact EIN and IRS object ID using the ProPublica full-filing pattern; that construction was validated across a repaired 603-period provenance matrix, including 110 exact target pairs; automated structural comparison found no numeric change and no mutation outside the four permitted citation fields. Independent release audit and production browser verification remain required before promotion.
Remaining limitation: ProPublica is a third-party filing viewer. The original IRS URL is preserved for audit, but this release did not re-fetch all 671 target destinations over the network.
2026-09-17 UTC · Hospital-record discovery and audience paths
Release: cpm-findability-v1-20260917T1648Z. Affected pages: patient prices, hospital staff, communities and quality-care committees, hospital lookup, and hospital records.
What changed: the public site added three audience paths, a name-or-ZIP hospital finder, clearer ownership and reporting-scope labels, price-scope and hidden-result disclosures, and direct routes for staff and community review.
Evidence and verification state: the release kept CMS hospital identifiers and IRS filing relationships source-linked and passed its automated release checks. This entry does not claim that every national comparison displayed elsewhere has completed row-level source registration; those exceptions are identified on the sources page.
Remaining limitation: exact facility-to-parent filing coverage is incomplete for some hospitals, and filed-year records do not establish current policy or conduct.
Correction reports use only these public states:
- Received: the report was received but has not been evaluated.
- Under review: the cited page and source are being compared.
- Corrected: a verified change was published and is linked from the entry.
- Closed without change: available evidence did not support changing the published material; the reason is recorded.
Do not expose a reporter’s identity, email address, patient information, internal credentials, private file paths, or security-sensitive details in an entry.
